Cardiac Rehabilitation

Our Cardiac Rehabilitation Program promotes optimum recovery and health for those recovering from heart disease and/or cardiac-related surgery. Our goal is to limit the physical effects of cardiac illness and improve cardiovascular fitness and overall health. Each guest receives a program designed specifically for them, which is then supervised and implemented by a team of cardiac specialists, including physicians, nurses, physical and occupational therapists, and consulting nutritional and psychological professionals.

Program Features:

  • Consultations by a board certified Cardiologist
  • Specific protocols for the management of patients with Congestive Heart Failure, Post Coronary Artery Bypass Surgery, Myocardial Infarction (Heart Attack), and LVAD’s
  • Specially Trained Staff in Cardiac Diseases
  • On-Site Coumadin Management, Labs, and Diagnostic Testing
  • Physical, Occupational, and Speech Therapies available up to 7 days a week

Orthopedic Rehabilitation

Symphony at The Tillers offers our guests a comprehensive Orthopedic Rehabilitation Program with all the therapeutic, clinical and technical resources needed to address a wide range of orthopedic conditions, including:

  • Joint replacement
  • Fractures
  • Cervical and lumbar injuries
  • Arthritis and osteoporosis
  • Amputations

Using a team approach to care, we utilize individual therapies, pain management, strength, balance and mobility training, biomechanical re-education, joint protection and other techniques to restore our guests to their optimum level of functional independence.

Wound Care

At Symphony at The Tillers, a holistic plan of care is designed for our wound patients. Our staff is trained to anticipate and care for their needs.

A Nurse Coordinator, assigned to each patient, works closely with family and physician to monitor progress and serve as a resource to address concerns. They collaborate with the Discharge Physician’s hospital to ensure appropriate follow up care is coordinated upon the patient's discharge from us.

Our interdisciplinary team, Physician Specialist, Wound Ostomy Nurse, Dietitian, Social Worker, Physical Therapist, Occupational Therapist, Speech Therapist and nursing staff assess and reassess the plan of care. This has reduced our average recovery time and stay by 2- to 8- weeks.
